Follow these steps:

1.- Review the Contrato de Cuenta Corretaje Bursátil (Brokerage Contract).

2.- Download Annex I of the Contrato de Cuenta de Corretaje Bursátil (Forms or Ficha de Identificación del Cliente).

3.- Download also The CVV (Custody Entity) Form.

4.- Send the Forms duly filled and singed along with the following documents:

1. Copy of the I.D. or Passport. In case you are married then you must send your Spouses I.D. or Passport.
2. Copy of a Bank Refference Duly Singed and sealed by the bank in which must appear the complete account number.
3. Copy of the valid RIF.

2.- Download Annex I of the Contrato de Cuenta de Corretaje Bursátil (Forms or Ficha de Identificación del Cliente).

3.- Download also the CVV(Custody Entity) Form.

4.- Send the form duly filled and signed along with the following documents to our office:

• Copy of the I.D of the Directors and Share Holders.
• Copy of a Bank Refference Duely Singed and sealed by the bank in which must appear the complete account number.
• Copy of the Articles of Incorporation, Certificate of Incorporation and Share Certificates including all the modifications.
• Copy of RIF (Venezuelan Registry of Fiscal Information)
